Exploring an idea around the 3rd root cause of failures I've seen
in working in a more agile way, incentives. Many times, if the
behavior and reasons don't make sense to you, take a look at how
the person is being rewarded, or the lack of rewards, and see if
you can understand it better. This can also be the secret key to
changing how people work, change their incentives. If you have seen
examples of this working against you, or altering them to work for
